Building a new Pharmacist team within a PCN with a list size of approximately 75,000 patients

The Problem

PCD was approached by a Primary Care Network (PCN) comprising five surgeries and serving approximately 75,000 patients. Having never previously integrated a pharmacist into their practices, the PCN sought PCD’s expertise to develop a pharmacist led support service. Through collaborative consultations and the strategic guidance of PCD’s directors, a highly effective workforce was mobilised carefully tailored to meet both the overarching needs of the PCN and the unique requirements of each individual practice.

PCD’s Solution

As there had never been any pharmacy support within this PCN, a trial was conducted at one of the surgeries, providing five hours of pharmacist support per day. The trial proved immediately successful, demonstrating a direct impact from day one, which led the PCN to commit to a full service. Each surgery was subsequently provided with pharmacist support proportionate to the size of its patient list size, resulting in a total of five full-time equivalent pharmacists across the PCN, including a lead pharmacist who oversaw the service. The pharmacists also collaborated directly with the ICBs to support local KPIs and ensure safe prescribing. Due to the efficiency and effectiveness of the service, the PCN opted to extend their partnership with PCD through the extended access service, enabling pharmacists to conduct medication reviews for a set number of hours each week.

Further Support

After 18 months of successful operation, the service was further enhanced with the addition of two pharmacists and two pharmacy technicians. In addition, the PCN also secured support from two full-time advanced clinical practitioners.
With the COVID pandemic and the roll out of the vaccines, PCD played a crucial role in the deployment of pharmacists to assist with the clinics ensuring oversight of the vaccines.

Conclusion

To date, with increased PCN funding, PCD provides the network with seven full-time equivalent pharmacist prescribers, two pharmacy technicians, two full-time equivalent ACPs, and extended access across each surgery. The service has been developed and built from the ground up to meet the specific needs of the PCN, ensuring an efficiently run operation while fully addressing all requirements of the network.
